PARIS UPDATE 2008 FROM DR. ZDINAK
In keeping with our mission statement and commitment to provide our Manhattan patients with the most advanced, international medical and laser-surgical aesthetic solutions, I’m pleased to report back to you regarding my involvement in the 2008 World Congress on Aesthetic and Anti-Aging Medicine that took place from April 10-12 in Paris, France. This annual conference is attended by 5,000 physicians from around the world who share the spirit of advancing scientific knowledge. The days were intense and exciting, filled with non-stop lectures, live patient demonstrations, and product vendors from around the world. We will be bringing new, cutting-edge treatments to our New York practice including mesotherapy, as well as new uses for the hyaluronic acid products Restylane and Perlane, and techniques for better delivery of vitamins into the skin. The meeting concluded with a gala dinner at the Paris Opera where we made new friends from France, Brazil, Russia, London, and Hong Kong. We will keep you posted as each of these anti-aging strategies is introduced so that you can benefit from the best and the latest that the world has to offer in the field of aesthetic medicine.

Say goodbye to dark under eye circles, stretch marks, and that unwanted underarm fat that can make your favorite evening dress look less than perfect! MOVE INTO THE SUMMER SMOOTHLY WITH PIXEL LASER RESURFACING
Did you know that the eyelid skin is the thinnest on the body, and that it is the first to show the signs of age? There have been tremendous advances in laser and light sources for the treatment of fine lines and crepe-like, heavy, loose eyelid skin and under eye bags. Dr. Zdinak is one of the few physicians in Manhattan offering “Eyes by Thermage” radiofrequency skin tightening. “Eyes by Thermage” employs a specially designed eyelid treatment tip that firms and tightens loose eyelid skin in a single, no downtime, in-office treatment. Results are immediate and continue to improve for up to six months. Fine lines and crepe-like lower eyelid skin can be smoothed away using the Pixel 2940 fractionated laser handpiece from Alma lasers. The pixel laser uses fractionated Er:YAG technology to gently resurface the lower eyelid skin with less downtime than the traditional CO2 laser skin resurfacing. The lower eyelids will appear slightly pink resembling a mild sunburn for one day, and then lightly browned for approximately four to five days. A series of four to six treatments at three week intervals will give you smoother, more youthful appearing skin around your eyes.
PERLANE TO THE RESCUE
At last, Perlane is here! Perlane, the sister product of Restylane, has been available on the international market since 2000 and has recently been approved by the FDA to reduce the appearance of moderate to deep facial creases and folds. Like Restylane, Perlane is made of hyaluronic acid, a natural sugar component found throughout the body. As skin ages, it loses volume and the ability to make more hyaluronic acid. Perlane is made of larger gel particles than Restylane, allowing for more filling capacity, ideal for deep nasolabial folds and for adding volume. Perlane can be administered in a simple office visit using a topical anesthetic. Because hyaluronic acid attracts and binds water molecules the treated areas are “plumped up” to take on a fuller, more youthful appearance. Perlane breaks down naturally in the body, and its benefits last for approximately six months to a year. No skin testing before treatment is necessary. BROWN SPOTS BE BANISHED WITH INTENSE PULSED LIGHT (IPL)
Even after you stop regular sun exposure, dark spots on the face, chest, and hands can still form years later. Intense pulsed light (IPL) therapy can restore a more even skin tone by precisely targeting the darker pigment with a focused wavelength of light, which travels directly to the melanin in the dark spots causing them to fade. First, a cold gel is applied to the areas to be treated, and then the skin surface receives a warm flash of light, which causes the dark spots to darken for a day or two, and then begin to lighten. Four to six treatments spaced three weeks apart provide significant improvement with no downtime.

Question: I feel as if I’m plagued with dark circles under my eyes! Is there anything that can be done to improve them?

Dark under eye circles are difficult to conceal with makeup, and often make you appear tired, even when you’re fully rested. This problem is relatively common but it’s important to understand the origin of dark circles in order to develop a treatment plan. Dark circles are typically caused by one of three factors, either alone or in combination. The first possibility is that there is actual deposition of pigment into the skin of the lower eyelid. This pigment deposition is commonly due to post-traumatic hyperpigmentation, where the skin of the lower eyelids has been perpetually irritated, usually from chronic tearing due to allergies or dry eye. Skin pigmentation is the easiest cause of dark under eye circles to treat because it responds well to a combination of carboxytherapy and home bleaching regimens. I do not use hydroquinones, the over-the counter bleaching agent, on delicate eyelid skin because in some individuals this can worsen the dark circles. Instead, I prescribe a combination of kojic acid and arbutin to help reduce the hyperpigmentation. This gives my patients a safe and gentle treatment product that they can use at home while the carboxytherapy sessions are performed in my office.

The second cause of dark under eye circles is vascular pooling. The capillary network of the lower eyelids can become congested for a variety of reasons. Normally, the tears drain from the eyelids into the nose, but if there is some obstruction due to chronic nasal congestion from seasonal allergies or a nasal fracture, the drainage doesn’t function well, and the blood flow to the lower eyelids becomes sluggish, giving rise to the boggy blue tinge known casually as “allergic shiners.” The lack of appropriate oxygenation to the lower eyelid skin allows the bluish cast to show through the thin skin of the eyelids. Carboxytherapy works to improve the capillary network of the lower eyelids, as well as to increase the dermal collagen layer in the lower eyelid skin. By injecting a small amount of carbon dioxide gas into the affected areas, blood flow is increased and improved capillary networks are formed for longer lasting circulatory benefit. The bluish cast is replaced with a healthy pink tone. Once a series of four to six sessions is completed, the skin has a more luminous appearance that lasts approximately six months.

The third major cause of dark under eye circles is a depression beneath the lower eyelids called a tear trough deformity. Tear trough deformity is a commonly inherited trait in the African American and Indian communities, but it can also develop in Caucasians over time due to the normal loss of bony architecture in the inferior orbital rim, which is the top of the cheekbone area. In the event that a tear trough deformity exists, a temporary filling agent such as Restylane or Juvederm can be placed in the valley to plump up the depression, which improves the appearance of the dark circles. The treatment is not painful, and it is a one-time procedure performed after a small amount of numbing cream has been applied to the skin. Since my specialty is oculoplastic surgery, I perform this procedure in my office at least twice a day with optimal results that last approximately eight months to one year.

Now there’s no need to live with dark circles. With the appropriate application of at home topical bleaching agents, carboxytherapy, and hyaluronic acid fillers, great strides can be made towards improving under eye discoloration.
